Terms and Conditions for Services

1)Reliable results are dependent on the adequate specimen collection, proper transportation, storage and processing. Compromised samples due to improper identification, delayed delivery (aged specimen), incorrect storage or handling, inappropriate container, or insufficient sample volume/quantity could lead to sample rejection.
2)Test results are intended for doctor’s reference only and should only be used in conjunction with patient’s clinical data. It is strongly recommended that these results should be communicated to the patient in a setting that includes appropriate counselling. The test results are only applied to the samples as received.
3)Based on the test need, the laboratory might refer samples and personal data to relevant testing institute for referral testing.
4)Unless for the reporting on critical result or result associated with public health concern, the laboratory would normally contact you through your referral physician. The laboratory could report test related information to relevant regulatory or accreditation body, if such reporting is required by law or accreditation requirement.
5)The laboratory recognizes its responsibilities in relation to the collection, holding, processing, use and / or transfer of personal data under the Personal Data (Privacy) Ordinance (Cap. 486) (“PDPO”). Personal data will be collected only for lawful and relevant purposes and all practicable steps will be taken to ensure that personal data held by the laboratory is accurate. The laboratory will take all practicable steps to ensure security of the personal data and to avoid unauthorized or accidental access, erasure or other use. Your personal identifiers, including name, identification numbers, contact details and any other test relevant information will be kept confidential and will not be shared with any third party unless being subjected to the conditions as stated in item 3 & 4. 6)Test data (excluded personally identifiable information) and the specimen will be retained by the company and may be used for future research to further increase the accuracy of the test.
7)Your personal data will be retained for not less than four years. If you want to delete your personal data, please apply to the company in written format.
8)If these terms and conditions have any discrepancies between English version and Chinese translation, the English version shall prevail.
9)In case of any dispute, “Pangenia Lifesciences Limited” reserves the right of final decision.
10)The signing on the test request form or other agreement format by the referral physicians and/or the test requestors reveals the acknowledgement and consent on the above terms and conditions on the test service.
